Please be sure to bring your insurance card to your appointment, as we will need to make a copy for our records. If you are an existing patient, it is important that our records reflect your most current insurance information.
CO-PAYMENTS AND DEDUCTIBLES
All co-payments are required at your time of service. We accept most traditional forms of payment; include cash, check, debit cards, MasterCard, Visa, American Express, Discover, and CareCredit.

SportsMED will submit charges from your visit to you insurance carrier(s). Our billing department will be happy to answer any questions you may have regarding high deductible payments, payment plans, or questions regarding a bill. You can contact our billing office at 256-881-5151 ext. 4, Monday through Friday between 8:00am and 5:00pm.
WORKERS’ COMPENSATION
If you are being seen under a workers’ compensation claim, it is important that we have your claim number, billing address for the claim, date of injury, and the name and phone number of the claim’s adjuster prior to your appointment. If you cannot provide this information by your appointment date, or if you have questions regarding your claim, please call Nikki Thomas at 256-705-4148.
REFERRALS
We recommend that you check with your insurance provider to see if you insurance plan requires a referral to see a specialist. If a referral is required, please notify your primary care physician to obtain a referral prior to your appointment at SportsMED. Your primary care physician can fax the referral to us at 256-704-0878, or you can call us with the referral information at 256-881-5151.
